DESCRIPTION:National Healthcare Decisions Day - Join the movement towards educated and informed healthcare planning and decision making!\n\n	\n\n	Claremont - VNA Hospice & Palliative Care of Southern California and its community partners will host a community forum in recognition of National Healthcare Decisions Day\, Tuesday\, April 16\, 2013 from 10 am to 2 pm at Pomona Valley Health Center Claremont\, 1601 Monte Vista Avenue\, Claremont.  \n\n	 \n\n	The forum will include a panel of experts from VNA Hospice & Palliative Care of Southern California and Pomona Valley Hospital Medical Center who will\n\n	answer and clarify questions regarding healthcare decisions as well as encourage conversations within their own families.
