DESCRIPTION:REAL Connections\n\nCelebrating Two Years of Bringing the Community Together\n\n \n\nREAL Connections\, the Claremont based\, village style approach to aging in place\, is celebrating its 2nd anniversary.  The idea that people want to live and age well in their homes and communities by accessing convenient services and resources with one phone call has really taken off since its inception in 2012.   REAL Connections believes "it takes a village" to remain living at home\, and for that reason they connect community members to lend a helping hand.  It's a safe way for people age 50+ to access concierge services\, trusted resources for transportation\, social events and trips\, help with day to day activities\, pre-screened home maintenance providers\, case management\, and assistance with hospital to home transition.  It is the discovery of warm new friendships\, and the satisfaction of sharing time with those in need\, that makes this concept a winning one.  \n\n\n\nA member based program\, REAL Connections (REsources for Ageless Living) relies on volunteers to assist with a wide variety of tasks. Volunteer opportunities range from driving someone to a doctor's appointment\, to walking the dog\, changing smoke alarm batteries\, or running a quick errand. Connecting with those in your local community\, and making friends along the way\, is simply a bonus of the program!\n\n\n\nOn Thursday\, July 17\, REAL Connections will host a picnic at Larkin Park in Claremont to celebrate the 2nd anniversary of serving seniors in the Inland\, Pomona and San Gabriel Valleys.  The annual summer celebration will take place from 5:00   7:00 p.m.\, with food\, music and fun for the whole family\, as well as the greater community.  Join REAL Connections as they celebrate the members in their community and the volunteers that help make a difference in their lives.  Don't forget to check out our photos from past social events!!!
