National Healthcare Decisions Day - Join the movement towards educated and informed healthcare planning and decision making!
Claremont - VNA Hospice & Palliative Care of Southern California and its community partners will host a community forum in recognition of National Healthcare Decisions Day, Tuesday, April 16, 2013 from 10 am to 2 pm at Pomona Valley Health Center Claremont, 1601 Monte Vista Avenue, Claremont.
The forum will include a panel of experts from VNA Hospice & Palliative Care of Southern California and Pomona Valley Hospital Medical Center who will
answer and clarify questions regarding healthcare decisions as well as encourage conversations within their own families. Each attendee will receive an advanced care directive, POLST form, The 25 Documents Everyone Should have in Place and the nationally acclaimed Conversation Project starter kit.
